The Washington County Wellness Initiative recently launched their Community Assessment survey. The assessment, which is done every five years, is important in helping WCWI in assessing community needs.

The survey takes approximately 15 to 30 minutes to complete and does not have to be completed at one time. The survey must be taken before April 15th. Answers are secure, anonymous and compliant with medical privacy laws.
